Vacancy Details :
Junior Staff Nurse
- Qualification : 10+2 with diploma in Jr. Staff Nursing training from SMF or any other recognized institute; Age upto 45 Years
ANM/MPHW(F)
- Qualification Matric with diploma in FMPHW from SMF or any other recognized institute; Age upto 45 Years
Adolescent Health Counselor
- Qualification PG in Sociology/ Social Work from any recognized university; Age upto 45 Years
Dialysis Technician
- Qualification M.Sc Renal Dialysis from recognized institute with one year experience in dialysis unit attached with hospital having minimum hundred beds Age: Upto:45 Years
Pharmacist (ISM)
- Qualification : Matric with diploma in pharmacy training course from SMF or any other recognized institute Age: upto:45 Years
Application Process
- Applications are invited for eligible candidates residing within the municipal limits/jurisdiction of District Health Society Kupwara. The positions are contractual and temporary under National Health Mission (NHM).

i. Domicile Certificate.
ii. Date of Birth Certificate (Matriculation Certificate) from recognized University.
iii. Degree/Diploma certificate of requisite qualifications sheet indicating marks of all the years of the requisite qualification sheet for all years/ one consolidated mark sheet/ India/State Medical Council/J&K state Nursing and Marks sheet of all past applied for.
Experience Certificate duly attested by Head of the institution/department (if applicable).

Under APMC Council attested by Director Health Services/domicile should be enclosed with Revenue Department Category for various Sub Centres of District), application forms unmarried Female candidates). Form under APMC certified issued by competent authority (Tehsildar). In case authority (Tehsildar) should be accompanied with Unmarried Certificate duly issued by competent authority (Tehsildar).
