- Public Health Nurse / Rehabilitation Nurse 01 B.Sc. Nursing / GNM
- E C G technician 01 12th with Science or above Qualification with a Diploma in X–ray Assistant with three months of specialized training in ECG from a recognized institute/SMF.
- Psychiatrist Social Worker 02 Graduation with sociology as one of the subject ( withdrawn)
- Tuberculosis and Chest Disease health Visitors 02 12th with Science or above with Diploma in relevant line
- Speech Therapist (ENT) 01 12th with Science or above with Diploma in relevant line
- Radiotherapy technician (for every treatment unit) 02 12th with Science with two years radiation therapy technologist course or equivalent

a) Date of birth certificate.
b) Domicile certificate.
c) 10th class Marks sheet
d) 12th class Marks sheet
e) Diploma / Degree from any recognised institute.
f) Marks sheets of all years /semesters along with final marks sheet indicating marks of all the years of the Degree / Diploma, issued by the concerned institute.
g) Registration certificate from JK paramedical Nursing Council. h) Experience certificate, if any.
• No documents shall be entertained after the last date of submission of application form. The candidates must possess all the qualification at the time of applying for the post.
• Candidates has to submit fee of 200 per application form. In case any candidate is eligible for more than one post he / she has to fill the application form separately along with fee.
